ANNEX 4 – GOVERNMENT-FURNISHED PROPERTY COMPLIANCE REPORT
Per NMCARS 5245.103-73(ii), HCAs are required to report quarterly on compliance with inclusion of GFP clauses in solicitations/contracts. The following format is required for reporting. An excel version of this sheet is available with the posted NMCARS 13-07 change at: http://www.secnav.navy.mil/rda/Pages/NMCARS.aspx.
Solicitation/Contract #: Insert the contract number.
Mod/DO/TO. Insert the modification number, delivery order (DO) number or task order (TO) number as appropriate. If the action is a modification of a DO or TO, insert both the DO/TO number followed by the mod number.
Program Name. A short description of the program name the GFP is supporting.
Contracting Officer (PCO) Name. Insert the PCO name.
Period of Performance Start date. Insert the POP start date.
Period of Performance End date. Insert the POP end date.
FAR Clause/DFARS Clause [XXX]. Indicate if the clauses were included in the contract reported in Column 1, as required by FAR and DFARS. Use only the following phrases; Included, Not Included, or Not Applicable. When “Not Included” is used, provide an explanation for the contract file as to why the required clause was not included in the contract, modification, DO or TO.
